Responsiblities
Patients are responsible for the following:
- Providing JBH Medical, to the best of your knowledge, accurate and complete information about present complaints, past illnesses, hospitalizations, medications, and other matters relating to your health
- Reporting unexpected changes in your condition to JBH Medical
- Advising JBH Medical whether you understand a planned course of action and what is expected of you
- Following the treatment plan recommended by JBH Medical
- Keeping your scheduled appointments, and if, for any reason, you are unable to honor that appointment, please notify JBH Medical within 24 hours, or a fee of $25 will be incurred
- Ensuring that the financial obligations of your medical care are fulfilled at the time service is provided
- Following JBH Medical protocols affecting your care and conduct
